Grants - Family Assistance Program
The purpose of the Family Assistance Program is to provide financial assistance to caregivers of persons living with Alzheimer's disease or another type of dementia. The funds are to be used to purchase supplies and services to aid the caregiver and the individual and is available statewide. Covered services include:
Respite Care, which offers a break for the caregiver and a change of environment for the person with dementia. Respite care can be provided through adult day care, in-home care, or a residential care facility (assisted living or nursing home).
Adult Day Care offers a day program usually Monday through Friday and occasionally on Saturdays. There are two types of adult day care providers. The first is a health model where a registered nurse and/or certified nurse assistants deliver the services. The social model offers services similar to a senior center but with a more structured program of activities. Both models usually offer noon meals and snacks.
Home Care services are provided directly in the home and may include home delivered meals, house-keeping services, sitter services, and personal care.
Residential Care Facilities, some RCFs, depending upon space available, will admit an individual for temporary care.
Legal Services include assistance from an attorney who specialized in elder law such as guardianships, durable power of attorney, and living wills.
Home Care Supplies/Modifications include products or services that assist with the care of the patient in the home. This can include purchasing incontinence supplies or Ensure, or modifications to the home such as installing grab bars in the bathroom or a wheelchair ramp.
Mental Health Services provide one-on-one counseling for caregivers who are having difficulties coping with caregiving or coming to terms with the illness. Service providers may include psychiatrists, psychologists, or licensed therapists.
In order to be eligible to receive financial assistance, an applicant must not receive the same services funded by an insurance policy, Medicaid, ElderChoices, Supplemental Security Income or Medicare. The patient must also have a dementia diagnosis from his/her physician.
