DHS Lifespan Respite Grant

The Arkansas Lifespan Respite Coalition has chosen our organization to distribute grants throughout the state for caregivers needing respite.

Respite care is for providing a caregiver a break and this $350 grant is for respite care only. It can be awarded to one care recipient (patient) once per calendar year (Jan 1 – Dec 31). This grant is available for anyone of any age, with a diagnosis of ANY special need or chronic illness that requires a caregiver.

The care recipient (patient):

• Can be of any age
• Must reside in Arkansas
• Must have an official diagnosis of any special need or chronic illness that requires a caregiver on a physician’s official letterhead or prescription pad

The amount of financial assistance given per grant approval is $350. A family may receive this grant once in a calendar year (Jan 1st to Dec 31st). For questions, or to request an application, call 501-224-0021, download an application here, or email [email protected].

The application must be completed, signed, and include a diagnosis of any special need that requires a caregiver on a physician’s official letterhead or prescription pad. It may take up to 10 business days to process your application.

An eligible application does not guarantee approval. Funds are limited to the number of processed applications submitted during the grant period. Alzheimer’s Arkansas cannot guarantee the availability of funds throughout the entire grant period.

This grant may be used to pay for respite care services ONLY. This includes services such as:

• in-home care
• temporary daycare
• short-term facility stays

After receipt of grant check and use of funds, applicants must submit the enclosed Respite Survey Log no later than 90 days after the approval date. This survey can be mailed to Alzheimer’s Arkansas at 201 Markham Center Drive, Little Rock, AR 72205, emailed to [email protected], or faxed to 501-227-6303.

FAQ

  1. Do I need to send in a DX with the application?
    Yes! The doctor can fax it or email it to us. All our contact information is notated at the bottom of the process page and application. The diagnosis must be dated within a year of submitting the application.
  2. What can this grant be used for?
    A short-term break for the unpaid family caregiver. The care provider must be over the age of 18 and not live in the same home as the patient (neighbor, family member, friend, etc). Or a professional care provider/company can be hired. It is the responsibility of the family caregiver to pay the hired provider.
  3. Can I apply for multiple grants?
    Only one grant can be open for each care recipient (patient) at any given time.
  4. When is service paid for?
    The check for $350 will be mailed to the caregiver applying with the approval letter and respite log. Respite service must be provided after the grant approval date.
  5. What if do not complete the application completely?
    Only fully completed applications will be processed. If any information is missing OR a diagnosis was not submitted, the application will be returned back to the caregiver for completion.
  6. What if I do not return the respite log?
    The respite log must be returned to our office within 90 days of the approval date. Failure to do so will result in the caregiver and patient not being eligible for future grants within our organization.

DHS Lifespan Respite Grants are limited to the amount of funds available. This mini-grant project is funded through the Arkansas Lifespan Respite Coalition and the Administration on Community Living.
